ISSUING PRESCRIPTIONS AND MEDICAL CERTIFICATES

Prescriptions for drugs treating illnesses other than those of psychiatric nature are issued based on the patient’s current medical records (certificates issued by doctors, hospital discharge forms, medical files)

Should a need arise, the following may be issued after a psychiatric examination:

• ZUS ZLA forms (doctor’s leave)

• certificates for ZUS concerning rehabilitation or annuity benefits

• certificate describing the condition of the patient’s health, issued

• for disability committees

• medical certificates for Psychological and Pedagogical Counseling

• Centers (e.g. confirming the need of special or tailor-made schooling curricula)

• referrals to hospital
